Becoming a CALM Client
Thank you for choosing me as your therapist. I look forward to working with you! Here's what I would like you to do before your first appointment:
- If you want to become a new client, or want your child to become a new client, please email me. Emailing is better than calling because I usually can return emails quicker than I can return phone calls. If you would like me to call you to discuss your request for counseling, I can arrange a time to call you via email.
- Download and fill out the New Client Forms. Your signature is required on the forms, so if you electronically return them to me you will need to either scan the documents or take a photo of the forms and attached them to an email (please read the "Electronic Communication Waiver" before sending any confidential information by email). If you have concerns about email security, you can mail the forms.
- If you need me to consult or share information with other professionals then please fill out the Consent for Release of Information.
Things to Remember:
Fees: My clients pay at the end of each session with checks or bankcards. The fee is $125 for a 50-minute session, $187.50 for a 75-minute session, and $250 for a 100-minute session.
Please arrive for appointments on time and take a seat in either the front or back waiting area. I will step out of my office and find you in the waiting area when it is your appointment time. While you wait, please make yourself comfortable. The waiting area offers toys for kids and books and magazines to read. You can help yourself to tea in the armoire. The restroom is clearly marked; just look for the sign on the door. Since I share an office space with five therapists, please be as quiet as possible in the waiting area.
Cancellation Policy: I prefer 48-hours notice when clients cancel. Since I often have a waitlist, this allows me enough time to refill any openings. I charge my full rate for cancellations made within 24-hours of an appointment.
